You don’t need proof of address, immigration status, ID or an NHS number to complete the form. This service is designed and run by the NHS, so your personal information will be kept safe.
Alternatively, you can register by coming into the surgery and requesting a new patient registration pack or complete the GMS1 registration form.
The Avicenna Medical Practice serves the whole of BD3, and areas of BD2 (if the second part of your code begins with 2, 3 or 4) and BD1 (if the second part of your code begins with 3, 4 or 5) BD4 (if the second part of your code begins with 0, 6, 7, 8 or 9).
If you are living away from home but do not want to change your home GP surgery than you can receive emergency treatment from any local GP practice for 14 days or contacting 111 service.
After 14 days you will need to register as a temporary patient. To register you’ll need to fill out a temporary services form (GMS3), available from GP surgeries.
After 3 months, you’ll have to reapply to register as a temporary resident or become a permanent patient.
Registering with a GP surgery outside your local area
You can register permanently with some GP surgeries outside your local area. This can be helpful if you want to use a GP surgery near your work instead of your home or prefer the services of a GP surgery that’s further away.
Registering with a GP surgery outside your local area can affect the NHS services that you can access including:
- home visits and urgent care from your GP, including out of hours care
- community services, such as physiotherapy and midwife appointments
Anyone, regardless of nationality, residency or immigration status, may register and consult with a GP without charge.
Being registered with a GP practice may mean you’re invited for NHS screening services, but you may still have to pay for these services when they’re not provided by the GP practice.
Also being registered with a GP practice does not in itself mean you’ll be entitled to free NHS hospital treatment. Further information for overseas visitors.
