This is an annual survey, which is usually sent out every January. We would greatly appreciate it if you could complete the form and then send it back as the results reflect on us. The more people that complete the form the better feedback we can get on our quality of services.
This will not only help us provide a better quality of care it will also improve other local NHS services in your area.

We make every effort to give the best service possible to everyone who attends our practice.
However, we are aware that things can go wrong sometimes resulting in a patient feeling that they have a genuine cause for complaint. If this is so, we would wish for the matter to be settled as quickly, and as amicably, as possible.
To pursue a complaint please contact the practice and speak to our practice manager who will be happy to help and will deal with your concerns appropriately and thoroughly. Further written information is available regarding the complaint’s procedure from our reception. Complaints leaflets are on the reception desk for patients to access easily.
